How is it calculated?

DPMO = defects / (units × opportunities) × 1,000,000; sigma is derived from yield via the normal distribution (with the 1.5σ shift).

6σ = 3.4 DPMO; industry average is 3–4σ. Each sigma level reduces defects roughly tenfold.
