Ishikawa (Fishbone) Diagram: Visualizing the Root Cause

A root-cause tool that lays out the possible causes of a problem by category, systematically.

What it is and the 6M

The Ishikawa diagram groups the causes behind an effect (problem) in a fishbone shape. In manufacturing the classic categories are the 6M: Man, Machine, Method, Material, Measurement and Milieu (Environment).

How to use it

Brainstorm with the team to list possible causes under each category, then go deeper with '5 Whys' to reach the most likely root cause. Ishikawa is a strong start for 8D's D4 (root cause) step and before an FMEA.
