The RACI Matrix: Clarifying Roles and Responsibilities

What is RACI?
RACI defines four roles for an activity: Responsible (does the work), Accountable (answers for it — a single person), Consulted and Informed. Tasks are rows, people/roles are columns, and each cell gets R/A/C/I.
Why it works
Unclear ownership is the most common cause of delays and conflict. By requiring exactly one 'Accountable' per task, RACI makes ownership explicit and reduces duplication and 'I thought you had it' situations.
